Title:
Patient Services Coordinator


Responsible to on PCN identified line manager behalf of SHCP:Responsible to on Practice Manager behalf of the practices


Location:
Surrey Heath Primary Care Network (Quatro House, Lyon Way, Frimley
Road, Camberley, Surrey) - The 'Patient Support Centre' and one or more nominated GP Practice(s).


Hours per week:
Full or Part Time hours, up to 37.5hrs per week (full time), Monday

  • Sunday 08:00
  • 20:00.

Salary:
£10.50 per hour during the training and Probationary period (6 months).

Following training / probation the role will be Agenda for Change Band 3 equivalent; from £21,730 to £23,177 per annum for full time, depending on experience.

Pro-Rated for Part Time.


Working Pattern:
The role requires working a range of shifts, across Monday to Sunday and across the hours of 08:00
  • 20:00.

Holiday: 27 days per year (based on full time hours) plus Bank Holiday entitlement

ROLE SUMMARY:

An exciting opportunity has arisen for Patient Services Coordinators to join Surrey Heath Primary


Care Network [PCN], supporting local Practices with patient contact to access GP services, and to assist with the achievement of the new Network Contract Direct Enhanced Service [DES] specifications and responsibilities.


We are looking for Patient Services Coordinators to work with our PCN, local Practices and the wider multidisciplinary team in coordinating key activities for patients including access to services, advice and information, and ensuring health and care planning is timely, efficient, and patient-centred.

You will support multiple local GP Practices, which will involve working in a central 'Patient Support

Centre' as part of a hub team. During your training period you will be expected to work directly from one or more local GP Practices.


You will be part of the 'front door' for patients and will support access to services and continuity of care by helping patients interact and engage with the right support at the right time and from the right health care professional.

You will manage and respond to patient and Practice initiated contact, including incoming and outgoing phone calls with patients, reviewing and responding to e-Consults, providing reception capacity and associated administration.

You will use triage and signposting skills to direct patients to the right support or appointment type to best meet their needs.

Key Responsibilities and Primary Duties may include the following:

  • Working as part of a team to fulfil a rota of Patient Services Coordinators, requiring you to work shifts across all 7 days of the week, between the hours of 08:00
