Key Working Relationships:

  • Patients Health & Well-being Team Social Prescribing Team members Health & Well-being coaches Practice management and admin teams Clinical Leads for the PCN MDT members including but not exhaustive: Clinical Pharmacists, Pharmacy technicians, District Nurses, LARCH Team, OPMH, IRT, Adult Social Care, Paramedics, Social Prescribing Link Workers External community network (e.g. CSE organisations, local authorities, other agencies)

BNSSG network of peers Purpose of the Role:

The Health and Wellbeing Team Leader will undertake the duties of a Social Prescribing Link worker, plus oversee and coordinate the work of the Health and Wellbeing Team.

The aim is also to reduce reliance on primary care where patients can affect changes in managing their own health and wellbeing.


Team Lead Responsibilities:
Co-ordinate the work of the Social Prescribing team ensuring that the work is appropriately distributed and prioritised.

Line-manage the Social Prescribing link workers and Health & Wellbeing Coaches including overseeing their personal development. Act as a source of knowledge and mentor and to the Health and Wellbeing Team.

Monitor the activity of the Health and Wellbeing Service and provide regular reports to the PCN and practice management teams.

Promote the vision and aims of the service to colleagues and external contacts.

Provide guidance to those referring into the service.


Social Prescribing Link Worker Responsibilities:

Social prescribing empowers people to take control of their health and wellbeing through referral to non-clinical social prescribing link workers.

The Link Workers give people time to focus on what matters to me and take a holistic approach to an individuals health and wellbeing.

Take a whole population approach, working with a range of people who may benefit from social prescribing, including people who are lonely, have complex social needs, low level mental health needs and long-term conditions.


Help people to identify issues that affect their health & wellbeing and co-produce a simple personalised care and support plan.

Support people by connecting them to non-medical, community-based activities, groups and services that meet their practical, social and emotional needs, including specialist advice services and arts and culture, physical activity, and nature and green based activities.

Use coaching and motivational interviewing techniques to support people to take control of their own health and wellbeing.

Support development of accessible and sustainable community offers by working in partnership with VCSE organisations, local authorities and others to identify gaps in provision, and take a community development approach to enabling growth in community activities and groups.
