Job Category: Medical/Pharmaceutical/Scientific Job Location: London, Greater London, United Kingdom HFH Healthcare are now recruiting for a Nurse Case Manager to join our team supporting clients with highly complex clinical needs and their teams of carers in the South and South West area of London. including Surrey. This is a full-time permanent position, working out of our Head Office in Wimbledon. At HFH Healthcare we meet the growing need for complex care by providing specialist nurse-led services to adults and children in their own the homes, in and around London. Working in partnership with the NHS continuing healthcare teams, our enhanced level of specialist clinical support enables individual clients to remain at home even when the level of health care required is highly complex. Our services are provided through locality and specialty focused multi-disciplinary Hub Teams. Our individually-designed care packages are highly specialised, nurse-led and strongly outcome based. Working closely with our patients and their families, we fully identify their specific needs and the most effective ways to meet them. All our care is delivered in clients" homes by skilled and experienced Healthcare Assistants. They undertake an inhouse training programme and are assessed against inhouse competencies. Effective co-ordination of care staff, resources and logistics support is essential to the effective delivery of field and head office services. Our objective is to meet Care Quality Commission (CQC) standards, with the objective of achieving a minimum rating of \"Good\" within and across all Key Lines of Enquiry (KLOEs) and to be seen as the preferred provider for homecare clients requiring complex health interventions. ROLE PURPOSE As a Nurse Case Manager, you will be responsible for ensuring client"s receive high standards of care delivered by carer safety within your caseload. Reporting to the Regional lead Nurse and accountable to Director of Nursing and working in close partnership with the Care Coordinators, you will oversee the placing of the right care staff to the right client whilst supporting and developing carers working within your client case load to ensure high standards of care are delivered. Service delivery, quality and governance * Create, review and update client centred care plans and risk assessments for each client in a timely manner. * Be a visible support to clients, families and care teams in the community setting. * Liaise with multi-disciplinary community teams to establish and maintain bespoke care packages, finding solutions to clinical issues as they arise * Enhance Client"s experience by leading and delivering efficient and responsive service, engaging through, Nurse Case Managers and frontline staff to solve issues affecting quality and safety * Work with Regional Lead Nurse to support a smooth onboarding process for all new clients on your case load * Responsible for ensuring clients receive high quality evidence-based care. * Provide reassure to Regional Lead Nurse that carer team understand the care required and have the ability and skill to deliver quality care by means of regular update on clinical practice, evidence of competency and clinical supervision. * Participate in the HFH Virtual Care Round (VCR) review for all clients within the region at least once a month, more frequently when there are issues to be resolved. * Ensure any actions assigned to you at the Virtual Care Round are completed within the timeframe * Visit clients a minimum of 8 weekly to ensure needs are being met as planned, issues are being addressed in a timely and effective way and we are meeting / exceeding the client"s expectation * Escalate client safety concerns to Regional Lead Nurse and/or HFH Healthcare SMT in a timely manner. * Assist with managing and resolving clinical complaints raised by clients or their representatives, and support with investigations in conjunction with the Clinical Governance team. * Work with Regional Lead Nurse and peer group to ensure all clinical policies and procedures are regularly reviewed and documented. * Ensuring all clinical SOPs are implemented and followed * Participation in clinical oncall rota PERSON SPECIFICATION - Nurse Case Manager * NMC Registered Nurse (Adult or Paeds) with current valid PIN NumberICU/ITU/High Dependency skills or experience working with continuing healthcare patients with high clinical care needs (in a community setting would be an advantage). * Post-qualification experience in an acute clinical care setting working with patients with a wide range of complex needs. * Experienced in supporting and mentoring staff * Proven ability to analyse complex problems/ information from a variety of sources * Experience of managing and accurately compiling multiple patient records/updates on a daily basis * Evidence of continuing professional development. * Community Healthcare, especially working with CCGs/CHC Teams. * Complex and challenging care environments A full UK Driving Licence and use of own vehicle is essential for this role.
