Produce outstanding written reports based on your nursing assessments and observationsWork closely with the management team to develop the service, audit, and improve qualityAttend all training and engage in CPD to further knowledge & promote nursingUnder NMC guidelines, ensure that individual care plans are complete & maintained collectively with residents, relatives and other healthcare professionals.Administer prescribed medicines with Healthcare procedure and Healthcare Commission guidelines. Here at Cygnet Cedars Clinic w ewant the men in our care to make their own choices and live as independently as they are able to. To achieve this, our in-house MDT supports them with our myPath learning disability model, which offers a psychology, speech and language and occupational therapy led approach alongside psychiatry and nursingWithin Cygnet Cedars is a two bed step-down service, allowing for further community integration and focus on daily living skills. Each individual in our care begins a personalised care pathway that helps them develop necessary skills to leave our rehabilitation services and move towards a less restrictive setting.Our Service user profile consists of the following: Primary diagnosis of learning disability and mental health complex needsForensic history – May present with co-morbid presentationsChallenging behaviourPersonality disorderSchizophrenia, depression, bi-polar disorderDual diagnosisComplex needs and/or multiple diagnosesMay be detained under the Mental Health Act or informalMay have a history of repeated hospital admissions Cygnet Cedars has been rated as “Outstanding” by the CQC.
