Description:Job descriptionMy client in East London urgently requires an experienced SWE registered adults social worker to join there Well-being and Prevention Team. The team will provide a first response for vulnerable adults and carers where a referral has been made and/or a request for care and support. The team will:� Receive referrals and requests for care and support from the screening and triaging function� Prioritise safeguarding concerns in line with the Safeguarding pathways and process� Deal with safeguarding concerns and/or enquiries� Contact the person to start the initial conversation within 24 hours� Provide social work support � Manage the equipment and assistive technology contract(s)� Provide out of borough equipment to support hospital discharge� Complete an initial light touch assessment (unlikely in most instances to be a full Care Act Assessment) where required within 48 hours to determine and understand presenting needs and if required determine eligibility� Develop and agree well-being plans� Facilitate access to community-led initiatives for an agreed length of time in line with the well-being plan � up to 6 weeks� Monitor and evaluate the effectiveness of well-being and preventative support� Review progress against any agreed plans and outcomes and evidence and record the outcome of interventions� Provide advice and support around suitable accommodation and tenancy options� Provide advice and support around volunteering, vocational and employment opportunities� Referral to appropriate services if there is on-going need that has turned into eligible need i.e. Independent Living Team Objectives:There are several objectives the WPT are trying to achieve:� Ensure vulnerable adults are protected from abuse and neglect� Support effective hospital discharge for out of borough patients� Increase the number of people who are able to self-serve and self-manage their care and support needs without needing the Council through information, advice and signposting and available online toolsets� Manage referrals effectively such as:o Safeguarding concernso Provision of small aids and adaptations (under �1000) to support daily living o Request for care and support for people with low level care and support needs (non-eligible)� Ensure people have a quick response when a referral and/or request for care and support is made � a conversation takes place within 24 hours and a visit/light touch assessment within 48 hours� Maximise people�s strengths and assets and those of the community and partners to meet needs themselves or through informal care and support � connecting people to support in communities and those provided by the voluntary, charitable and third sector and checking to see this has resolved the enquiry� Provide advice and support around suitable accommodation and tenancy options� Support people earlier in their adult social care journey in a person-centred way to promote strengths, assets, wellbeing and prevent escalation of need through light touch managed support and products � pausing assessments and trying out products and services to see if they have resolved the enquiry/request� Provide appropriate equipment and assistive technology solutions to meet needs and better understand and manage risk� Based on the information from the conversation and light touch assessment either:o Provide information, advice or signposting and closeo Provide small items of equipment or adaptations to support daily livingo Provide an independent living offer pack e.g. assistive technology pack, dementia concern packs, preventative equipment pack etc.� Develop and agree with the resident a Wellbeing Plan � using the Wellbeing Planner tool provided by the Council. The wellbeing plan will:o Identify the areas where support to maintain and/or improve wellbeing is neededo Outline how long support will be provided for� Provide practical support to people in an agreed way to manage their low level wellbeing and prevention social care needs and meet outcomes important to them� Provide short-term social work support to help people and carers manage risk and resolve any care and support needs lessening the need for more intensive interventions such as:o determine capacity and best interesto appointeeships/deputyshipo advice and support around protection of propertyo food bankso house cleans/deep cleanso no recourse to public fundso strategies to manage risk � reduce the number of people who develop eligible needs and may need formal short-term care and support i.e. reablement� Evidence the effectiveness of our wellbeing and preventative initiatives and supportMultidisciplinary working:The WPT will work in a multi-disciplinary way to ensure we can maximise a person�s wellbeing and prevent social care needs worsening or escalating.
