Description:If you are looking for an exciting and challenging job why not join the Community Virtual Ward an excellent opportunity has arisen to expand our team of Experienced Health Care Assistants to provide delivery of care for patients in their own home with long term conditions, palliative care and rehabilitation needs across West Norfolk. Also to provide enhanced care to patients in their own homes up to 72 hours or until infection has reduced. You will be working within a team to facilitate early discharge from the acute setting and also admission avoidance. Referrals also received from Community Matrons, General Practitioners. You will need to be able to demonstrate effective communication skills, problem solving skills and be resilient and able to cope in changing and challenging situations. You will work under the direction of Registered Nurse but will also be required to work autonomously. Previous experience in health care as a Health Care Assistant in community setting would be desired but not essential.The Virtual Ward service runs from 08:00 – 22:00 seven days a week, it also covers night sits from 22:00- 07:00 seven days a week.Supervision will be provided by a band 6 Registered Nurse and band 5 Registered Nurse.
