Description

Working as part of a dynamic and passionate team you will support young and young adult carers (carers) aged to improve their personal, social, health and learning experiences and achieve equal access to fulfilled lives.

You will equip them to experience life as children, young people and young adults first, around caring.


You will provide one-to-one and group support work that improves skills, knowledge, confidence, resilience, life opportunities and support networks and prepare young people for adult life.


Functions specific to this role:


  • To ensure a coordinated approach for carers to reach appropriate support
  • Assess the needs of carers, linking them with community based and CTS services
  • Design and facilitate group support work and materials linked to this work
  • Undertake assessments, support plans and Reviews including Transition and Care Act compliant Carers Assessments
  • Offer personalised Support Work to carers in a variety of settings to address a focus of support and achieve desired outcomes
  • Attend relevant multiagency meetings to ensure effective partnership working
  • Offer personalised support to carers to access and/or remain in education, training or employment
  • To support transitions from children to adult services, education, training, employment and post caring
  • Provide drop in's at local educational settings

Job Types:
Full-time, Temporary contract


Salary:
From £22,863.00 per year


Schedule:

  • Holidays
  • Monday to Friday
  • Weekend availability

Experience:

Support Worker: 1 year (preferred)

Work Location:
Hybrid remote in Solihull
