Description

Due to expansion of the service, we are looking to recruit an RA Planner to work within Community Intermediate Care Service (CICS).

You will work within an office based team who plan and coordinate care for patients within their own homes.

Your role within that team will be to schedule care calls for the Rehabilitation Assistants to both Active Recovery and Stroke patients using an electronic system.

You will be responsible for the entry of patient data ensuring that appropriate care is delivered to patients by the most appropriate member of staff.


You will be expected to cover shifts from 6am to 8pm, 365 days a year including weekends and Bank Holidays.


  • To operate IT systems for allocating work to the RAs (including updating electronic patient record systems)
  • Assist the RA Coordinators in ensuring rotas are updated with sickness, training, annual leave etc.
  • To work in partnership with clinical staff to provide an effective and efficient care and rehabilitation service
  • To employ excellent communication skills
  • To provide a flexible approach to working hours to meet service demands.
  • To participate in CPD and maintain ongoing personal development.
  • To undertake any other duties that might be considered appropriate by the Head of Service after negotiation and adequate training.
  • To comply with the organisational and departmental policies and procedures.
