Description

The post holder will work as part of the Children's Community Nursing Team, in the delivery of support to families with children and young people who have additional health needs identified.


To work independently with delegated responsibilities for a defined patient caseload from the Children's Community Nurses, in range of settings appropriate to the child and/or young person e.g.

school, clinic or home.


To undertake planned assessments and clinical interventions following agreed care pathways and individual care plans, ensuring all deterioration or changes are reported to the appropriate Children's Community Nurse/Clinician.

E.g. Blood pressure monitoring, respiratory, cardiac, enteral and wound care.
