Community Nurse - Learning Disability

Sutton

£25/hr

Akton recruitment are currently working with a Local Authority in Sutton who are looking to recruit an experienced Social Worker to join their team.

This is a great opportunity for a long term temporary contract.

A great opportunity to join a friendly team of experienced Learning Disability Nurses, working in the London Borough of Sutton.

As an Learning Disability Nurse you will be working on a varied caseload from Continuing Health Care checklists and reviews, workwith Supported Living homes and individuals in their own homes.


Working to ensure all our clients have their health needs met and complete their health screening, you will be working closely with GP's and PCN's.

There are also opportunities to work with the varied Multidisciplinary team members in the Clinical HealthTeam for Adults with a Learning Disability.


About the role:


Our team collaborates effectively with Social Workers and Commissioners in Health and Social Care, providing assessment and/or treatment to individuals and families in supported living and residential placements who face barriers in accessing mainstream services,despite reasonable adjustments.

As a member of the Clinical Health Team, you will report to the Team Manager and work closely with Social Workers, Commissioners and other NHS teams to increase the uptake of health care services, including GP annual health checks, and reduce the need for emergencydepartment visits and hospital admissions.

Our team is committed to providing excellent leadership, offering regular operational and clinical supervision to support our staff. We also provide various training opportunities for ongoing clinical professional development.
