Role Purpose:

You will be the Senior Partnerships Officer, covering Nottinghamshire County for the Your Health Your Way Integrated Wellbeing Service.

You will be expected to engage with both operational and strategic stakeholders at community and place level to ensure true system integration across all elements of the service.

You will support the Head of Service and Specialist Leads to develop and maintain effective referral pathways into the IWS and build long
- lasting relationships with system partners from a range of sectors, including but not limited to: primary and secondary care, CVS, place based partnerships, local authorities and the ICB. You will be responsible for engagement within the South Notts locality and manage a team of Engagement Officers, each covering a different area of the county (Bassetlaw and Mid Notts) and the county-wide Digital Marketing and Communications Officer.


You will be required to conduct weekly 1:1s and team meeting, while monitoring referral targets and weekly updates reports and provide support/guidance around ongoing workstreams.

You will be a motivated, passionate, organised and proactive communicator, with experience working in a community setting. ABL take a strengths focused asset-based approach to community delivery, looking to innovate and partner, not replicate existing delivery.

The role will require dynamic working which embraces the vision and values we wish to establish for the IWS supporting people to achieve healthy lives.

No two days will be the same in this varied and flexible role.

If you are as comfortable spending a day with the team as you are working independently and confidently with new people and are highly organised this could be the job for you.

This is a demanding job, but you will not be on your own. ABLs highly experienced and friendly multi
- disciplinary team of Health and Social Care professionals, Business Intelligence, Service Development, Governance, Finance and Business Development are there to support.

Additionally, existing experienced delivery teams will be available to support you to drive the Service forward


Key responsibilities will be:

Design, monitoring and evaluation of a bespoke engagement strategy to promote the IWS Working with ABLs communication team to help raise the profile of IWS.

Engaging key system partners to explore partnership opportunities and co-production activities.

Working closely with The Head of Service and Operations Team Leads to manage the referral rates and develop partnerships with all potential referrers across Nottinghamshire.

Representing the service at key strategic meetings across the health and social care system. Developing and maintaining relationships with the three place-based partnerships and 7 local authorities. Attending internal senior leadership meeting to report on progress against the engagement strategy. Attending commissioner meetings to report on all engagement workstreams as required.

Contributing to commissioner reports and the internal performance review board as requested by the HOS. Chairing a weekly engagement team meeting.

Design, monitoring and evaluation of a bespoke communication strategy to promote the IWS Supporting the Engagement Officers to develop innovative pilots to support reduce health inequalities across Nottinghamshire.

Working with the HOS to manage the community fund.

Managing external projects such as the YHYW Community Champions and NHSEI projects.
