Job overview:

Salford is currently undergoing service transformation to redesign our Home First discharge processes and pathways. Our vision for Home First is 'People are in the right place, at the right time - meaning nobody spends more nights away from home than is absolutely necessary'


We are establishing a Home First Multidisciplinary Community Hub to support people admitted to hospital to be discharged 'Home First' and we are looking for a Band 7 Mental Health Practitioner to join the team.


Main duties of the job:


  • You will work as part of the Salford Home First Multi-Disciplinary Hub. You will provide specialist mental health advice to the Hub team and to hospital /Intermediate care unit staff for people who are medically fit for discharge requiring a support plan after discharge.
  • You will provide advice, supervision and consultation to multidisciplinary colleagues, on the support for the management of mental health issues.
  • Undertake specialist mental health assessments, develop formulations, offering highly developed risk assessments and management plans.
  • Provide colleagues with assistance in navigating Mental Health, Psychological Therapy services and 3rd sector as onward pathways.
  • Be expected to provide support and training to the acute hospital staff and the Home First Hub regarding recognising mental illness and appropriate onward actions to be taken.
  • Liaise with mental health services across Salford to support post discharge planning and implementation.
  • Attend and contribute to Best Interests meetings

  • RMN, BSc Occupational Therapy or equivalent
  • Professional Degree D32/33 or ENB998

Knowledge, Skills and Experience:

  • Trained Clinical Supervisor
  • Qualifications related to physical health related problems
  • To be a nonmedical prescriber (NMP) or be willing to undertake this training
  • Significant post registration NHS experience including experience of working in all core clinical areas
  • Evidence of wide clinical theoretical knowledge base
  • Well developed communication skills
  • Ability to work as part of a team
  • Experience of working as part of an integrated MDT
  • Flexibility
  • Good organisational and time management skills
  • Car Driver
  • Experience of working in the community and/or rehabilitation
  • Basic IT skills including: Log in and Log out, Operating a mouse and keyboard and Navigating a Windows based environment
  • To demonstrate an awareness and understanding of supporting equality and valuing diversity within their role
