We require a passionate, enthusiastic, reliable and hardworking individual to join our team as a Patient Pathway Coordinator in the Pain Management Unit for the Pain and Long COVID team.


We are looking for an excellent and proactive team player who can offer commitment in providing support to clinical and non-clinical staff.

The post holder will be the point of contact for all administrative issues relating to patients' pathway of care. The post holder has responsibility for co-ordinating and organising the activities of the Multidisciplinary Teams (MDT) and team meetings.

This will include liaising with all the departments that are involved in providing care and core services to diabetes patients.


The post holder will ensure high levels of patient and clinician satisfaction by being an accessible, customer focused and knowledgeable point of contact.

To provide a consistent approach across the Trust, using Standard Operating Procedures (SOPs) to ensure that functions of the role are carried out correctly within given timescales.

The post holder will be the point of contact for all administrative issues relating to patients' pathway of care.

The post holder will work as part of a multi-disciplinary team and will provide expertise in the proactive management of the patient pathway from referral to discharge, ensuring the entire pathway is managed smoothly.

The post holder will supervise and allocate work to the team.
