Norfolk and Waveney ICB Digital Team has a vacancy for a Digital Change Facilitator to work on the Shared Care Record Project.

This project brings patient records from different systems together so that clinicians have the information they need, where and when they need it.

The post holder will work within a team on the development and delivery of technology to staff across the Norfolk and Waveney Integrated Care System, empowering process change through technology and being a champion of using systems and information.

The post holder will support the Digital Projects Manager and the wider project team by providing effective Change management for the Shared Care Record (ShCR).

The post holder will be a digital change champion and will be instrumental in establishing a new culture of positive change through technology and data sharing whilst providing support and guidance, digital expertise, enthusiasm and solutions.


Specific duties include:

  • Providing support to staff within the ICS in the adoption of the Shared Care Record and their new ways of working
  • The design and delivery of change programmes to enable the achievement of agreed goals and benefits, connecting with staff and patients/service users to facilitate and prepare them for this transition
  • Improving adoption and usage of the ShCR by encouraging and promoting the adoption of the ShCR, acting as a digital champion and utilising change management methodologies


Health and care services in Norfolk and Waveney are working closely together to further improve services and provide more joined-up care for local people.

In Norfolk and Waveney, we have already achieved a lot by working in partnership to improve health and care outcomes.

These changes have been made possible by different organisations - NHS hospitals, GPs, mental health and community health services, local councils, care homes and social workers, voluntary and community organisations and others - joining forces to agree and plan for local people's needs.


Norfolk and Waveney Integrated Care System (ICS) includes a statutory Integrated Care Partnership (ICP), and an Integrated Care Board (ICB) called NHS Norfolk and Waveney.

This partnership and organisation dedicated to making sure that organisations work together for the benefit of our residents, staff and communities is an important step change, helping to create positive differences to local people and joining up health and social care.

This is the culmination of many years of effort to build partnership working across the NHS, local authorities, the third sector and patient groups.


The main purpose of the post is to be a Digital Change agent, an enthusiast and enabler; you'll be a presence in the clinical environment and instrumental in the effective delivery and adoption of the Shared Care Record.


  • To understand the current business processes identified for each project and be the main facilitator working with colleagues to develop and agree future business processes to meet both operational and project requirements. This will include aligning processes to the agreed benefits of the project
  • To lead and arrange workshops with relevant levels of staff to develop new business processes as required. This will include developing workshop material including presentations to facilitate discussions with staff and reach agreement on how new processes should work. The role will involve ensuring that identified governance controls are met where changes are introduced
  • To map "as is" and "to be" processes in order to facilitate the change for project delivery
  • Advise and report on training requirements arising from project implementation, through the production of the relevant project and business change documentation
  • Receive and provide complicated information verbally, in writing and electronically to large groups of staff. Developed interpersonal, communication, persuasive and motivational skills will be required when delivering presentations, awareness, training and advice sessions to individuals or groups, in order to overcome sometimes opposing views to change and gain acceptance or understanding of national and local requirements and resulting implementation challenges and change in processes
  • Provide support to project and service leads through a benefits and business change management process to support day to day practice, performance management and service development, facilitating, enabling and managing the successful change elements of the ShCR project
