Utopia Care are looking to recruit an experienced Care Coordinator to join our team at our new branch soon to be opened in Thurrock.

RESPONSIBILITIES:

Answering telephone enquiries
Support the Registered Manager in the recruitment of carers.
Processing new client referrals
Preparing appropriate documentation for assessments and completing them with clients
Allocation of carers to clients and ensuring all visits always covered
Administration of all Individual person centred care plans
Maintaining client and office files and ensuring that they are always up to date
Ensuring all documentation and records are kept up to date at all times
To be flexible and able and willing to complete care visits that have not been covered
To ensure compliance with all office systems and procedures
Participation with the on call phone on a rota basis
Production of reports and management information as required
To complete audits as required

Part time contract available 16-20 hours per week with the opportunity to increase over time

Up to £13 per hour
