At Ashton Court, we are seeking an experienced and highly skilled individual to join our team as a Care Planner.
As a Care Planner, you will play a critical role in coordinating and organising care services for our residents, ensuring their individual needs are met with utmost professionalism and compassion.
Furthermore, excellent people skills are essential for this role. You will be working closely with our care team, residents, and their families, as well as other healthcare professionals.Effective communication, empathy, and interpersonal skills are vital in building rapport, establishing trust, and maintaining positive relationships with all stakeholders.
A solid understanding of the Mental Capacity Act is also a requirement for this role.As a Care Planner, you will be responsible for ensuring that all care plans and decisions comply with the legal requirements and ethical principles outlined in the Mental Capacity Act.
This includes assessing and documenting the mental capacity of residents, involving them in decision-making processes, and advocating for their rights and autonomy.
