An exciting opportunity has arisen to join our community training team based in Aylesbury and High Wycombe.

As part of the Bucks Community Learning Disability Team we provide training in person centred approaches and support the development of enabling environments designed to enhance the quality of life of service users.


We are seeking a passionate, energetic and highly motivated practitioner with excellent communication skills and extensive experience of working within a PBS framework and the development of workers in front-line community services.

As a PPT Coordinator the post holder will provide long-term support for people with complex needs by;

  • Supporting people with a learning disability and challenging behaviour to live in welcoming and supportive communities.
  • Providing personalised, flexible and responsive support to assist individuals, families, services and communities to access accurate and timely information to clarify their goals, strengths and needs.
  • Supporting the delivery of PPT's training programmes and providing opportunities to embed the learning into the day to day.
  • Assisting people and their supports/services to manage barriers and
constraints in achieving their preferred lifestyle.

  • Monitoring the quality of services and supports and the implementation of established support plans and protocols.
  • Supporting the development of inclusive communities through partnership and collaboration with individuals and families/carers, local organisations and the broader community.


The Positive Partnerships Team promotes collaboration and partnership working with service users, their families, paid carers and all agencies that are involved with people with a learning disability.

We aim to provide an innovative, safe and progressive service. Our focus is always on the quality of life of service users and their support networks.


You will be part of a team of specialists who work creatively, collaboratively and compassionately to deliver the very best standards of care helping us to achieve our aim of building the right support for people with learning disabilities who may be autistic, have additional health needs and/or challenging behaviour.

Coordination

  • Build and maintain effective working relationships with service users, their carers/families and communities in a defined local area.
  • Support access to accurate, timely and relevant information and to assist individuals, families and communities to access information through a variety of means.
  • Provide individuals and families with support and practical assistance to clarify their goals, strengths and needs through the development of individual care and person centred plans.
  • Support access to effective independent advocacy and selfadvocacy
support as required.

  • Assist individuals, carers and families to use personal and local community networks to develop practical solutions to meet their goals and needs including access to health and social services.
  • In collaboration with the Service User and their family, monitor the quality of the service provided by the funded support services and raise concerns with the Learning Disability Health Team.
  • Collaborate with the other members of Learning Disability Health
Team to facilitate implementation of recommended intervention and
support strategies.

  • Where training and development needs are identified, support
access to PPT training programmes.

  • Ensuring effective risk assessments and risk support plans are in place to ensure that vulnerable people are kept safe.
  • Facilitate safeguarding process as required.
Community Development and Capacity Building

  • Develop and maintain a clear understanding of local community strengths, resources, connections, gaps and opportunities.
  • Develop partnerships with individuals, families, local organisations and the broader community to build a more inclusive community.
  • Build effective partnerships and working relationships with community and statutory services, including police, GPs, community organisations, health, adult social care, children and family services.
  • Assist local services to provide better support to people with a learning disability and challenging behaviour.
Practice Development

Administration and Information Management

  • Ensure uptodate information is maintained regarding contact and support provided to individuals and families, including the development, monitoring and review of individual plans within the Trust data system.
  • Provide information and data for reporting purposes and
requests for information as agreed with Team Leader

Professional Development and Supervision

  • Participate in an approved supervision and performance development process and undertake training and development related to the position.
  • Participate in the induction training of new employees and trainees
allocated to the team.
