We have an exciting opportunity for an Assistant Practitioner to join our growing Diabetes Team.
Based across the three main sites of Royal Derby Hospital, Queens Hospital Burton and Florence Nightingale Hospital Derby, you will work within the diabetes team supporting the Registered practitioners to provide individualised, holistic care in a safe and caring environment.
In discussion with the Registered Practitioner, the patient and significant others assess the immediate and ongoing needs of the patient and develop an individualised plan of care, implementing and evaluating care input.
Supporting and advising junior members of the team.
Collaborating and sharing information within the Diabetes team, UHDB team and where necessary with our Community Partners.
In discussion with the Registered Practitioner, the patient and significant others assess the immediate and ongoing needs of the patient and develop an individualised plan of care
In consultation with the Registered Practitioner prioritise, implement and evaluate patient care, in accordance with Trust polices, procedures, protocols and guidelines
Undertake a range of extended clinical procedures relevant to the clinical area, in support of registered nurses and to ensure the implementation of individualised care plans.
Venepuncture and Cannulation, clinical observations, management of insulin Pumps and retrieving data, use of Continuous Glucose Monitors and Hybrid Closed Loop technology.
Communicate effectively by giving, receiving and interpreting information so that appropriate action can be taken within the boundary of the role.
Work in partnership with all members of the interdisciplinary team both internal and external to the Trust to ensure continuity of care for the patient.
