Job Summary Intermediate Care Workers provide support to the day-to-day delivery of intermediate care in a community setting. They offer support in a patients own home or place of residence.

This care enables the individual to do the things that are important to them, and which are essential for their health and wellbeing.

Intermediate care services are provided to patients after leaving hospital or when they are at risk of being sent to hospital.

This service could help prevent patients going into hospital in the first place or a residential care setting unnecessarily.

This is a new role aligned to the Horizontal Project which aims to deliver care for vulnerable patients to avoid unnecessary admissions to hospital or residential settings.

The funding for this project comes from the Integrated Care Board as has been secured for 2 years.

Please note, you will not be required to undertake any personal care or administration of medication as part of this role.


Key responsibilities and tasks You will: Support and contribute to the population health project. Providing practical and emotional support (telephone calls, welfare checks, shopping etc) and guidance to patients in the home to prevent an attendance or admission to hospital. Providing practical and emotional support in the patients own home and signposting to other services where appropriate to enable the individual to live independently in their own home. Providing reactive, focused, and flexible support to adults either following a hospital stay to enable early discharge when clinical needs have been met or to those living in the community to prevent hospital admission.


Assisting the Health and Local Crisis Response team by undertaking duties required to meet the patients needs to re-enable patients to remain in their own home and prevent hospital re-admission.

Responding to referrals from Clinicians and Community Health Professionals and you will be required to have the ability to assess patients needs, complete and follow a support plan for the individual.

Other Be willing to undertake travel to various locations to carry out duties of the post as required.

To safeguard the health, well-being, and safety of the patients we work with, some of whom maybe classed as vulnerable people or adults at risk.


In the event of a risk to a Patient becoming apparent or if concerns arise about a vulnerable persons welfare, to immediately report these concerns in line with the appropriate policy and procedure.
