The Expert Patients Programme (EPP) a free 6-week self-management course for adults living with long-term health conditions

The Wandsworth Wellbeing Hub an online directory and telephone signposting service

The Diabetes Champions a group of volunteers, trained by Diabetes UK to educate people about the risks and management of Type 2 Diabetes.
