We have an exciting opportunity for a project lead to help us with the roll out of Patient Stratified Follow Up for our cancer patients.

The post is for 18.75hours per week for 12 months fixed term, The post holder would sit within the cancer supportive care and wellbeing service and be expected to work across both Winchester Hospital and Basingstoke Hospital sites.


A key aspect of this role will be to manage the development of PSFU, providing assurance that the patients on remote surveillance pathways are monitored safely.

They will develop, with the site specific teams an implementation plan for HHFT to develop their new pathways.

To work with relevant stakeholders across HHFT to develop the implementation of cancer survivorship pathways and personalised care. The aim is to process map current pathways and determine how and when the stratification process and Recovery Package can be incorporated in order to redesign pathways of care

Develop, implement, and monitor a framework for quality and safety of the remote surveillance pathways

Ensure that any PSFU developments have patient involvement in the planning, implementation and evaluation.

(If a registered professional) Maintain adequate patient documentation to NMC requirements for all patients seen and advice given in any practice setting and collate and record clinical activity/data collection as required.

  • To work with relevant stakeholders across HHFT to develop the implementation of cancer survivorship pathways and personalised care.   • Develop, implement, and monitor a framework for quality and safety of the remote surveillance pathways
  • Be responsible for the regular monitoring, auditing, and reporting of compliance with the remote surveillance pathway in line with the governance protocols.
  • Ensure that any PSFU developments have patient involvement in the planning, implementation and evaluation.
