Job summary
We have an exciting new opportunity to work across the inpatient emergency portals and into the community as an Assistant Practitioner supporting the new 10-year health plan.You will be key in helping to deliver and develop this pathway to ensure that we support flow and the patient journey appropriately ensuring high quality care.
You will work in collaboration with the Physiotherapy and Occupational Therapy teams to support and develop the pathways from the emergency portals out into the community.
