An exciting opportunity has arisen for the position of a Technology Enabled Care Transformation Officer.

Our Service recognises the value and importance TEC and the opportunities it creates in transforming and enhancing people's choices and empowering them to live independently and for longer in their own home.


The role


In cooperation with our communities and partners, you will support greater access to our TEC Services, build our portfolio potential, create greater collaboration and partnerships and focus on delivering technology solutions for improving outcomes for our people we serve.


Main responsibilities


This role will be part of an expert team for introducing Assistive and Technology Enabled Care into support plans, ensuring the successful transition of service delivery from traditional to forward thinking and innovative service.


Support the workforce as the expert in practice and raise awareness and confidence, becoming an expert on assistive and digital enabled support.


You will be responsible for building and maintaining relationships with key partners in Adult Social Care and NHS whilst also liaising with community, voluntary and private sector.


We are looking for.

  • 4 GCSE ''C'' or above grades or equivalent in English and Maths and significant experience of working in an adult social care.
  • Knowledge of assistive and technology enabled care within NHS and Social Care
  • Experience of case management and personcentred planning and support
  • Excellent organisational and communication skills
  • Be commercially aware about TEC with the ability to creatively think and develop new ideas and ways of working.
  • Displays a positive attitude to change and is willing to work flexibly.
