Band - Emergency Assessment Uni Discharge Planner

The Emergency Assessment Unit at The John Radcliffe Hospital currently has a vacancy for a Discharge Planner.


We are a friendly team of skilled practitioners, who take pride in the quality of the care we provide in a fast-paced and demanding environment.

We know the values of providing excellent care to the most vulnerable patients, at times of high stress for them and their families.

We are looking to recruit a discharge planner who is motivated and proactive, with excellent communication skills. Experience working within a health care setting and previous knowledge of the discharge planning role is advantageous.


The successful applicant will be working with our team of experienced discharge planners who provide a 7 day service, supporting the medical and nursing staff with complex patient discharges.

We can offer a supportive environment with a strong commitment to professional development. We can offer full or part time hours, working weekdays and weekends.

  • To work as a member of the multidisciplinary team to support the discharge process.
  • Assist in the process of assessing and making appropriate and timely discharge plans for patients in the department.
  • Effectively communicate information to patient/relatives/carers and all members of the multidisciplinary team.
  • Encourage the patient's family and friends to be actively involved in the discharge process.
  • To record and document patient information.
  • To input patient data onto the IT systems.
