Crane Valley PCN Care Coordinator
RESPONSIBLE TO:
Clinical Director and deputy clinical director
HOURS: 37 hours (Monday - Friday however during the Covid-19 Vaccination
Programme flexibility is key as morning, afternoon, evening and
Saturday shifts will be expected)
LOCATION:
Working across GP Practices in:
Crane Valley PCN (Barcellos, Cranborne, West Moors and Verwood)
as well as at the Covid-19 Vaccination Centre.
FUNCTION
The role is a mixture of administration and care coordination, supporting teams and practices in delivering the key performance indicators of the Network DES and other key network service areas, but specifically, at least during the Covid-19 Vaccination Programme, the Network Care Coordinator will support administratively the requirements of the vaccination hub including; data entry, checking patients in, trouble shooting and anything else required to ensure the smooth running of the vaccination programme.
REQUIREMENTS OF THE ROLE
Covid-19 Vaccination Programme
The Care Coordinator will work closely and in partnership with those running the administration of the Vaccination Hub as well as Practice Managers.
- Be on hand at the hub when required to;
- Input vaccine data
- Call patients at short notice if they have not attended or if slots need to be filled
- Check patients in at the desk
- Guide patients to vaccination stations
- Ensure vaccinators and staff and volunteers have all they need to perform a highquality vaccination programme including refreshments and equipment
- Help in the car park where required.
- In practices help with the administration of;
- Inviting patients for their vaccination
- Contacting DNAs or cancellations
- Fielding and responding to vaccination queries from patients and practice staff.
- Moving latechange clinics and all the patients therein.
Other Duties
Each Care Coordinator will be aligned to a practice and will report to the Practice Manager and ideas about how the role will progress could include;
- Helping the Practice Manager in any patient related work on a daily basis
- Training in all areas of practice work to ensure cover during short notice sickness (prescriptions, workflow, secretarial.)

From £12.48 per hour
