Description
The post holder will work as part of the Community Swift Support Team within a multi-disciplinary framework.

individuals will have a diagnosis of a learning disability and or Autism who are presenting with a mental illness and / or severe challenging behaviour, whose presentation has moved away from baseline and/ or crisis.

Applicants must have a valid driving license and access to their own vehicle.


The post holder will be able to work with clients with the full range of mental health diagnoses who find themselves in a crisis and requiring specialist short term support as an alternative to hospital.

You will work very closely with the relevant teams e.g., CTPLD / UCAT / IST depending on diagnosis, who will provide clinical direction and support.

You will be proactive in seeking out the positive support networks available in the community for each individual which may help them in their recovery.


The post holder will provide direct "hands on care" and therapeutic engagement which could include the following: role modelling, implementation of structure, routine, communication systems, boundaries and intervention derived from assessment by the CTPLD/ IST/Mental health teams/CAMHS through the challenging behaviour care pathway.


The post holder will work alongside the Transforming Care Team, Intensive Support Team, Community Team for People with Learning Disabilities and with a variety of stakeholders such as client, family, service provider, social care, commissioners.
