About Age Concern Twyford & District:

Age Concern Twyford & District is a non-profit organisation dedicated to supporting the elderly population in Twyford and surrounding areas.

Our mission is to enhance the quality of life for older adults by providing a range of services, support, and advocacy tailored to their unique needs and preferences.

We are seeking a compassionate and dedicated individual to join our team as our Outreach Coordinator.

In this role, you will work closely with our Centre and Care managers to provide vital support and assistance to elderly individuals within the local and surrounding communities.

Previous outreach experience is preferred, but not essential.

Hours - Mon - Fri Additional hours may be required to accommodate earlier/later external presentations/meetings


Responsibilities:


  • To increase membership and promote the services of our Adult Social Club
  • Assist with the planning, coordination, and implementation of outreach activities and programs aimed at engaging and supporting elderly individuals.
  • Collaborate with local agencies, sheltered housing, social clubs, healthcare providers, and community organisations together with gaining an excellent rapport with the Elderly Unit at the Royal Berkshire Hospital, and Wokingham Borough Council to gain additional funded members
  • Build an excellent rapport and hold monthly coffee mornings with other medical professional organisations including the Dementia Clinic, Memory Clinics, District Nurses, Blind Society, Stroke Society & The British Deaf Association, GP Practice Managers
  • Conduct outreach visits to the homes of elderly residents to assess their needs, provide information about our available services, and offer a free taster session.
  • Assist elderly individuals in accessing community resources, social services, healthcare facilities, and other support networks as needed.
  • Carry out presentations to small groups detailing the services we provide
  • Growing the Adult Social Club membership by introducing people via advertising our services on Parish noticeboards, chemists, libraries, bowls clubs, British Legions etc
  • Assess the requirements of people, offering our lunch club facility should they not wish to attend all day.
  • Thinking outside the box and suggesting new ideas to gain new members
  • If people are isolated, to promote our small friendly social club environment, and set their minds at rest that we are a friendly caring team, providing emotional support, companionship, and encouragement for individuals facing loneliness, or other challenges.
  • To promote our take home evening meal service together with our hairdressing and chiropody facilities.
  • Maintain accurate records of outreach activities, client interactions, and service referrals in accordance with organisational protocols, including follow up and outcomes.
  • Identify cases that need assistance, and before taking action to discuss these with the Centre Manager/Care Manager
  • After discussion with the Centre Management, to inform the relevant services if a person is found to be in need of interventions from Social Services
  • Act as an emergency backup carer in the event of a staff shortage in the Social Club (although this should not detract from the task of obtaining new members)
  • Provide a signposting service for elderly people and provide advice to people in need.
  • Produce our weekly Friday Roundup newsletter, distributed to our members, their families and the surrounding community.
  • Participate in team meetings, training sessions, and professional development activities as required.

Qualifications:


  • Previous experience in outreach work, social services, or related field preferred
  • Excellent communication skills, both verbal and written
  • Strong interpersonal skills and the ability to build an excellent rapport with elderly individuals
  • Empathy, compassion, and a genuine interest in supporting the wellbeing of older adults
  • Ability to work independently as well as part of a team
  • Reliable, organised, flexible, and able to manage time effectively
  • A valid driver's license and access to a reliable vehicle is essential
  • An enhanced DBS check will be carried out before duties commence
