  • Social Work England Registration
  • Extensive Adults Social Work Experience
  • Experience of working with older people, people with disabilities, and long term health conditions and their carers.
  • Knowledge of the Care Act and Mental Capacity Act


This local authority based between London and Kent is seeking an experienced
Social Worker to be part of
Rapid Response Team. This authority will be offering you a laptop and mobile phone so you can work flexibly as well as from home when you have reports saving you time and money.


Rapid Response Team works closely with the A&E and acute medical units at two in hospitals in the area to minimise unnecessary hospital admissions and support early discharge from hospital where they can.

The team will assess your health and social careneeds and, if appropriate put together a package of care.


As a Social Worker in the team you will have an opportunity to work in a creative and strength focused way.

We are looking for Social Workers who embrace creative assessment and care planning working with informal support networks, understand community engagementand partnership working.
