Description
An opportunity has arisen to join the specialist services as a Forensic Discharge Pathway Facilitator.

The Discharge Pathway Facilitator will be an integral member of Lancashire and South Cumbria NHS Foundation Trusts (LSCFT) newly formed Specialist Services Secure Pathway Hub.

The Post Holder will work in close partnership with a range of agencies and organisations internal and external to LSCFT to ensure robust and streamline inpatient care pathways, take a lead role in facilitating discharge planning which will include early identification of barriers to discharge, taking action to prevent any unnecessary delay to discharges, facilitating complex multi-agency case discussions in relation to discharge.
