Description

The role is to support vision impaired adults to maximise their independence and to provide support for their carers and families.


This will involve assessing for services and equipment, whilst also providing information and support to people who have sight loss.


The role requires enthusiasm and commitment, with strong communication skills and the ability to work independently within a multi-disciplinary team whilst using own initiative.


Job purpose


To undertake an assessment of needs and provide the appropriate support to people with sight loss to facilitate independence and improved health and wellbeing.


Appropriate support would include signposting, referrals to voluntary and statutory organisations, and joint working with health & social care professionals including appropriate support to carers and families.

As an outcome focused service all actions undertaken must be recorded in the appropriate IT systems.


Key Accountabilities

  • Work flexibly and collaboratively with colleagues and other organisations as part of the Essex Sensory Contract
  • Responsible for recognising a disclosure of a concern from an individual that requires a safeguarding intervention to be made, and to take appropriate action.
  • Undertake holistic person-centred assessments of Vision Impaired People (VIP) and provide appropriate information, advice, guidance, and assessment of their requirements, and refer on for appropriate services, rehabilitation, and equipment to meet identified needs.
  • Identify equipment that could be of benefit and order as appropriate.
  • Maintain a high level of knowledge of other services and equipment, including digital technology that people may be signposted to or informed of, to get further guidance and support.
  • Work with colleagues to maintain the ongoing development of services within the sensory service and across the organisation.
  • Maintain close links with voluntary organisations and partners, liaise with other specialists to maintain and improve relationships and services.
  • Maintain VIP records for all KPI requirements on Charity Log.
  • Manage own caseload effectively and meet associated key performance indicators.
  • Ability to make and implement appropriate decisions in relation to day-to-day queries and referrals and seek advice as appropriate.

Duties and Responsibilities

  • Data inputting and liaison with colleagues and other agencies.
  • Experience of completing assessments desirable but not essential. Experience of working with people who are sight impaired desirable (but not essential).
  • Ability to identify objectives and outcomes and the means to achieve them.
  • Strong communication skills and experience of / willingness to use a range of communication methods with customers and colleagues.
  • Able to develop networks and make use of contacts in an informal setting to meet the needs of service users.
  • Ability to liaise with statutory agencies, voluntary organisations and be able to communicate clearly, precisely, and professionally.
  • Confident using IT equipment such as computers, smart phones, databases, and relevant IT recording systems.
  • Able to plan efficient workload management including home visits and deskbased work.
  • To work under pressure and to demonstrate the ability to think quickly, soundly, and clearly.
  • Valid driving licence and car, experience of driving a van desirable but not essential.
Experience with dealing with sensitive data and understanding of Information Governance


Salary:
£23,500.00 per year
