We are seeking a dynamic and motivated senior leader for our new Head of Long-Term Conditions (LTC) and Population Health Management for the Suffolk and North East Essex Integrated Care Board (SNEE ICB) and wider Integrated Care System (ICS).

This is an exciting opportunity to be at the forefront of significant NHS changes and the delivery of these at a local level.

You will lead a team using your experience and knowledge of a broad range of long-term conditions across our Stay Well Domain, addressing issues of variation across the ICS and targeted change within place-based Alliance projects, involving **_Diabetes, Cardiovascular Disease, Respiratory, Stroke and Neuro

  • Rehabilitation, Long Covid, ME and CFS, Tobacco Dependency Reduction _**.. This role also introduces a single accountable lead for PHM to ensure SNEE benefits fully from the potential of PHM. For PHM the role will primarily be focused on developing and implementing the NECS Axym Platform and Linked Data Sets. The postholder will need expertise in programme design, pathway development, project management, data analysis and interpretation and stakeholder engagement.
