Social prescribing empowers people to take control of their health and wellbeing through referral to link workers who give time, focus on 'what matters to me' and take a holistic approach to an individual's health and wellbeing, connecting people to diverse community groups and statutory services for practical and emotional support.

The social prescribing link worker will work as a key part of the primary care network (PCN) multi-disciplinary team, to reduce health and wellbeing inequalities of the residents of St Helens.


By utilising an asset-based approach, the link worker will address the wider determinants of health such as debt, poor housing, mental health and isolation that can negatively affect health and wellbeing.

The post holder will work collaboratively with both lifestyle and wider statutory and community-based services to ensure the provision of effective and timely advice and support for individuals.

Ultimately, the link worker will provide a holistic approach to resolving issues and improving overall health, wellbeing and quality of life through helping individual's maintain or regain independence.
