Exciting opportunities have arisen for qualified Social Workers, to join Integrated Community Services (ICS) Team.

The ICS Social Care team works closely with colleagues in the ICS Health (community based) teams and alongside health colleagues within the hospitals.

The team provides a discharge to assess and admission avoidance service within Shropshire, operating with a home first and strengths-based philosophy, the team works to maximise independence and reduce reliance upon paid services where possible.
