We are currently looking for an experienced Health Care Assistant to join our small hard-working team. This role will focus on long term conditions management (CDM).This is an exciting opportunity to be involved in improving health in patients with long terms conditions.
With appropriate training you will be able to recognise lifestyle risk factors for common disease such as high blood pressure/cholesterol and diabetes.
You will also receive training on performing specialist chronic disease management and giving lifestyle advice.This is a fantastic role within the practice, and we would like you to possess the following skills.
- Phlebotomy
- BP
- Preferably wound care although not essential
- Please see job description for further essential and desirable clinical skills
The role will involve Admin work and the ability to recall/monitor CDM patients utilising our clinical system, Vision.
We are a 3 GP Partnership, including 2 Salaried GPs, 2 Practice Nurses, 1 Practice Mental Health Nurse, 1 Community Link Worker, Pharmacy Support, 1 Clinical PA, 5 Patient Care Advisors and a Practice Manager.
We work in a spacious, purpose build primary health care centre and have District Nurses, Midwives and Health Visitors are also on site.
