Sanctuary Personnel is currently looking for an experienced Band 6 Community Psychiatric Nurse (CPN) to take on a new ongoing contract role as Care Coordinator in the Adults Community Mental Health Team in Oldham.

Location:
Oldham, Greater Manchester.

Pay Rate:
£26 per hour (assignment rate).

Contract Type:
Locum, 6-months ongoing.

Main Duties:

To support with a caseload up to 25 and support duty on a rota basis.
Work with patients under CPA, MDT approach.
Care plan and review of care support needs from a health and social care perspective.
Work in a joint partnership team with Pennine and LA alongside supporting statutory duties and safeguarding manager and Senior Practitioner support available and supervision.

Requirements of the Community Psychiatric Nurse:

Degree or equivalent in Mental Health Nursing.
Experience working as a Mental Health Nurse.
NMC Registration.
Assessment, risk management and formulation skills.
