The successful Care Co-ordinator must be able to communicate professionally with clients and care staff to ensure the smooth running of the contract.


The Care Coordinators main duties include:

  • The organisation and co-ordination of care services in accordance with current best practices, according to policy and procedures, agreed standards, legislative requirements, and relevant regulation under the direction of the Registered Manager.
  • To organise, coordinate and supervise care services within the agency in accordance with agreed standards.
  • Systematically solve daytoday problematical issues which arise.
  • Ensure that care services are delivered to client at the times and for the duration specified.
  • Ensure that the staff allocated, and the staff attending, each client have the relevant skills, knowledge and resources related to the needs of the client.

  • Rota planning for staff: (required)
- domiciliary care (required)

  • Driving Licence (required)
  • Care coordinator experience in a domiciliary care setting (essential)
  • Level 3 NVQ in health and social care and working towards level 4 (essential)
  • ICT knowledge (Desirable)
  • Understanding of the 5 Key Lines of Enquiry (Desirable)

Schedule:


  • Monday to Friday
  • Weekend availability (be ability to work every other weekend)
