We are looking for an enthusiastic individual who can demonstrate they have experience of working with adults requiring care and support.
You will be required to work with adults over the age of 16 who may present with a wide range of care needs and also support and work with unpaid carers.
You will be required to adopt a person centred and outcomes focussed approach.In this role, there will be a focus on supporting people who are in interim care placements to return to their own home.
The offer of an interim care placement is made when a patient is ready for hospital discharge, but the Health and Social Care Partnership is unable to source an appropriate care at home package immediately, or when their first choice care home is temporarily unavailable.
You will be expected to give advice and practical help on the challenges associated with daily living activities.You will have strong assessment skills and will be able to plan, maintain, and monitor packages of care, as well as have the flexibility and skills to respond to changing needs of individuals.
Ideally, you will be able to demonstrate experience of multi-disciplinary working and be committed to working closely with colleagues from other disciplines.
