Trent Nursing are seeking Childrens Complex Community Support Workers in the Manchester area.Trent Nursing seeks to promote and provide care, education, support and opportunities for people with Mental Health, learning disabilities and autism in the community.

Our client has complex medical needs,Hours of work - our client is 18 months old - Altrincham area.
2 x nights (Sundays and Thursdays) and 3 x days (Mondays, Wednesdays and Fridays 11-5pm)
The Role:
Key tasks and responsibilities:
- To provide support to children with complex medical needs following their outcome based support plans.
- To adhere to the outcome based support plan in line with relevant policies and procedures and current legislation.
- To be actively involved in the delivery, coordination and having autonomy within the team.
- To ensure care is of the highest quality, demonstrating best practice and promote safeguarding.
- To ensure accurate and robust recording within the recording systems including online systems.
- Comply with Finance processes to safeguard individuals.
- To build collaborative relationships with internal and external partners to maximise the effectiveness of the service.
- To ensure all actions comply with safeguarding policies and procedures.
- To participate in regular review meetings.
Skills and Attributes
- To embed and demonstrate, Compassion, Contribution, Collaboration, communication and commitment in all aspects of care delivery.
- To develop a knowledge of mental health learning disability and other conditions as relevant to supported individuals.
- To work collaboratively with colleagues and all relevant parties, to deliver high quality support, whilst driving forward autonomy within individual teams.
- To be confident using IT and sharing information through digital processes.
- To be a skilled communicator, as well as understanding differences in communication and how this can be best supported.
- To contribute to and create support plans in partnership with supported individuals, their families and other stakeholders.
- To be creative, working on your own initiative to improve quality within services.
- To be flexible and adaptable in approach.
- Demonstrate a desire to learn, taking opportunities to expand your knowledge whilst developing your own reflective practice.
Knowledge and Experience
- To maintain mandatory training requirements and develop knowledge of mental health and learning disabilities in order to demonstrate continuous professional development.
- Must have Enhanced DBS registered with online update service.
- Previous experience working with mental health and learning disability services.
