Vacancies have arisen within the Continuing Care Service. The service offers home care/community support to children with complex medical needs.

We are looking to recruit enthusiastic team leaders to join the Continuing Care Team.

The Continuing Care Team provides specialist support for children/young people with complex medical needs, in the community.


As a Team Leader you will be involved in the clinical supervision and ongoing support of our team of Senior Health Care Assistants.

The role will involve cascading training to other team members and therefore the successful applicant must be prepared to undertake further clinical training themselves.

Responsibilities also include planning, implementation and evaluating care in conjunction with parents, carers and other staff members.

You will be working with children and young people aged 0 -19 years with various levels of dependency. All the children/young people you will be working with have complex health issues/severe disabilities.

The service provides bespoke care for a wide range of conditions which range from ventilated dependent young people, terminally ill children and others with severe disabilities/additional complex health problems.


CLINICAL

  • Planning, implementing and evaluating care plans utilising evidencebased practice.
  • Visit/contact families to offer support, advice and training.
  • Actively participate in teaching sessions to care staff.
  • Facilitate competencybased training for care staff.
  • Monitoring and evaluation of competencybased practice on a regular basis.
  • Clear and concise recording of competencybased practice.
  • Be actively involved in staff support offering clinical supervision to care staff.
  • Be involved in multidisciplinary meetings.
  • Liaise with other members of the multidisciplinary meetings
  • Act as a resource for clinical advice and support. This will involve oncall working

CLINICAL

  • Responsible for planning, implementing and evaluating all plans in conjunction with other staff members, parents and carers.
  • To maintain a high standard of care by utilising evidence and research based practice.
  • To ensure nursing practices are maintained within the Trust's Policies, Procedures and Guidelines. To also develop guidelines relating to the individual child's care needs.
  • To promote effective communication system, which ensures a team approach to family centred case is adopted.
  • To maintain accurate records in relation to patient care to meet the requirements of the guidelines for records and record keeping NMC 199
  • Participate in the training and teaching of all staff within the packages of care.
  • Monitoring reassessment of the clinical competence of staff.

MANAGEMENT

  • Accept delegated managerial responsibilities.
  • Be involved in clinical supervision and ongoing support of staff.
  • Be aware of economic and correct use/maintenance of resources.
  • Liaison with other agencies: Education, Health, Social Services and the voluntary sector.
  • Be involved in supporting and implementing of clinical risk management.
  • Be involved with performance review in accordance with Wirral Hospital's agreed policy.
Be involved in staff recruitment selection process.
