Description

Coach and motivate patients through multiple sessions to identify their needs, set goals, and support them to implement their personalised health and care plan.

Provide personalised support to individuals, their families and carers to ensure that they are active participants in their own healthcare; empowering them to take more control in managing their own health and wellbeing, to live independently, and improve their health outcomes through:

Providing interventions such as self-management education and peer support; and supporting people to establish and attain goals set by the person based on what is important to them, building on goals that are important to the individual.

Working with the social prescribing service to connect them to community-based activities which support their health and wellbeing.

Provide personalised support to individuals, their families and carers to take control of their health and wellbeing, live independently and improve their health outcomes, and in setting, meeting and achieving agreed goals.


This may include:
o Meet people on a one-to-one basis, making home visits where appropriate within organisations policies and procedures. Build trust with the patient, providing non-judgemental support, respecting diversity and lifestyle choices.

Work from a strength-based approach focusing on a persons assets o Be a friendly source of information about health, wellbeing and prevention approaches o Work with individuals to co-produce a simple personalised support plan to address the persons health and wellbeing needs Provide support to local community groups and work with other health, social care and voluntary sector providers to support the patients health and well-being holistically.

Ensure that fellow PCN staff are made aware of health coaching and social prescribing services and support colleagues to improve their skills and understanding of personalised care, behavioural approaches, and ensuring consistency in the follow up of peoples goals where an MDT is involved.


Raise awareness within the PCN of shared decision making and decision support tools and supporting people in shared decision-making conversations.

Work with people with lower activation to understand their level of knowledge, skills and confidence (their Activation level) when engaging with their health and wellbeing.

Explore and support access to a personal health budget, where appropriate, for their care and support.

Utilise existing IT and MDT channels to screen patients, with an aim to identify those that would benefit from health coaching Maintain accurate patient records at all times Networking, learning and development with fellow Health and wellbeing coaches across Liverpool primary care networks, sharing best practice initiatives.


Implement, maintain and embed a directory of services, incorporating referral processes to local services / organisations, user expectations and costs Supporting practice staff in the referral process, and in the identification of patients suitable for health and wellbeing coaching.

Encourage people, their families and carers to provide peer support and to do things together, such as setting up new community groups or volunteering.

Encourage people, their families and carers to provide feedback and to share their stories about the impact of health and wellbeing coaching on their lives.

Work closely within the MDT and with GP practices within the PCN to ensure that appropriate referral codes are inputted into clinical systems (as outlined in the Network Contract DES), adhering to data protection legislation and data sharing agreements.

Supporting people to engage in healthier lifestyles and behaviours through the use of health technology, specifically simple tele-health. Presentation of health education and information in a group work setting.

Facilitate group discussions, which could also provide tools for the individuals to use in the journey of lifestyle changes such as action plans.

Signpost to supportive services and other activities.


One to one consultation which could involve a health profile assessment, identification of health goals and action plan with supported action.

Signpost and support individuals to access services and activities, which support their healthy lifestyle changes such as exercise groups, etc.

In addition to the primary responsibilities above, the Wellbeing Advisor may be requested to:
Produce regular reports in relation to service delivery and progress Partake in audit as directed by the audit lead
