About the role:


As a care coordinator, you will be supporting and organising the day-to-day provision of high-quality care and support with administrative and scheduling responsibilities.


You will also be working closely with the Registered Manager to ensure service delivery is compliant with Care Inspectorate requirements.

Your office base will be in Perth and the role will include a mix of clinical and office duties.


Key responsibilities of our
Care Coordinator will include:

  • Ensuring care worker rotas are completed and sufficiently well planned, considering travel time and routes
  • Liaising with the clinical team regarding shadowing opportunities for new staff
  • Identify gaps in rota and liaise with recruitment team regularly to ensure recruitment of suitable staff so that all shifts are filled
  • Liaise as necessary with partner professionals and organisations (e.g. social workers, GPS, nurses) in respect of developing and delivering holistic, personcentred packages of care
  • Dealing with queries from clients and families in a calm and professional manner
  • Carry out client assessments and administration for all Individual care and support agreements
  • Ensure compliance with all Care Inspectorate requirements
  • Plan and allocate care assignments to appropriate care workers, with due regard for the particular needs of the individual and the skills and attributes of the worker
  • Carry out oncall coordinating and response duties outside normal office hours as agreed.

Skills required

What we are looking for in our Care Coordinator:

  • Experience of working in the domiciliary care sector with a good understanding of how the sector works
  • Must have knowledge of rostering, call logging and call monitoring systems
  • Ability to prioritise own workload and work under pressure
  • Great verbal and written communication and IT skills
  • Someone who has a caring attitude, is reliable/flexible, honest and a hard worker

This is a permanent full-time contract, and you'll be required to participate in the on-call rota. This is a role that requires being office based.
