JOB PURPOSE


The post holder will provide Carers support, engagement and information events on behalf of BCC, working flexibly as part of a multi-agency Community Hub Centre.

The role will support improved collaboration between agencies, driving forward best practice around the Carers agenda.

The key objectives of the role are to:

  • Work across multi-disciplinary teams to provide timely information and support to carers and their families.
  • Work with our Time Out Team to support carers in respite sessions and activities to develop friendships and skills.
  • Support carers in managing their caring role and maintaining their own wellbeing through proactive practical and emotional support.
  • Promote awareness and identification of carers across professional services and within the community through information events and training facilitation.
  • Work collaboratively with relevant agencies to ensure a holistic approach to support is in place.
  • Provide effective, outcome focused reports in line with funder requirements to demonstrate that identified objectives are sufficiently evidenced.

KEY TASKS AND RESPONSIBILITIES

  • To demonstrate Passion, Quality and Respect in all aspects of the role.
  • Based across Community Hubs and Blackpool Carers Centre as an integral part of multidisciplinary teams identifying carers and their families who require support.
  • To provide verbal and written reports as required, including statistical data and qualitative feedback to demonstrate that the aims and objectives of the project are being met.
  • To provide intensive, one to one emotional support where appropriate.
  • To provide awareness raising and information sessions for professionals.
  • To maintain an effective database and accurate records for adult carers and information relating to them.
  • To maintain an uptodate knowledge of local services and provision in order to navigate carers and their families to appropriate support.
  • To undertake assessments with carers and families using Blackpool Carers Centre assessment tools and refer to statutory or other voluntary organisations, including Blackpool Council for a Carers Assessment where appropriate.
  • Carry out effective consultations with carers and the people they care for ensuring that all services are developed in line with carers' needs. Promote their active involvement and participation in developing local services.
  • Work in partnership with existing providers to ensure carers and the person being cared for has access to appropriate support or representation.
  • Attend and participate as required in health, statutory and voluntary sector groups, events or meetings.
  • To liaise regularly with the Adult Lead Worker and Adult Carers Director, attend meetings and provide written and verbal reports indicating the progress of work and identification of areas where further development is required.
  • To maintain excellent administration and recording
  • To work effectively with BCC colleagues regarding external communications.
  • Attend training courses and events as required.
  • Contribute to the coordination, training and support of volunteers and students within BCC. Work closely with volunteers and students.

OTHER DUTIES

  • To maintain excellent administration and recording
  • To work positively and flexibly as part of the team to ensure the smooth daytoday running of services for carers
  • To attend and participate in regular internal staff meetings
  • To undertake other tasks as required.
  • To work under the direction of the Adult Carers Director and Adult Carers Lead; adhere to all Carers Centre policies and procedures and represent the Carers Centre at appropriate meetings.

Up to £21,500.00 per year
