Reporting to the Home Manager or Lead Clinical Nurse and Registered Nurses.

The Care Coordinator is responsible for ensuring a high level of care is provided to our residents and meets the code of conduct and company procedures.

The Care Coordinator should have pre-existing high level knowledge of safe and effective medication administration. The Care Coordinator should have proven pre-existing experience with care team management.


The Care Coordinator will support the Registered Nurses, Clinical Lead Nurse and the Home Manager providing leadership to the care team and providing strong / specialised knowledge to ensure the highest standards are achieved in all areas of the home, including relationship centered care and clinical care.


JOB SPECIFIC RESPONSIBILITIES

Leadership and Management

  • Responsibility for assisting in care/specialised care in the home.
  • Ensuring training targets are met on each of the units and the teams are competent in their roles.
  • Lead, support and inspire the care team to deliver outcomes for the home.
  • Responsibility for the supervision, appraisal and performance management of the care team, supporting them to achieve their potential.
  • Support the leadership, of the care team, to ensure they are aligned with the Opus Care Ltd vision
  • Initiate regular communication with colleagues, residents, relatives, and external stakeholders maintaining confidentiality and data protection at all times.
  • Ensure that all regulatory and statutory requirements are met and company policies
and procedures are adhered to.

  • Ensure any requirements of a care perspective from internal and external audits are actioned within the timescales required.

Care / Specialised Care

  • Support the leadership of care delivery in the home, to ensure continuous assessment, planning, implementation and evaluation of resident's care.
  • Responsibility to uphold care compliance in the home.
  • Ensure that an outstanding level of relationship centered care and care/specialised care is being delivered within the home.
  • Ensure care plans are regularly audited and that they are appropriate to the residents needs and reflect the care being delivered.
  • Ensure safe and effective medication administration in line with the NICE Guidelines.
  • Ensure medication is ordered in line with company policies and procedures and managed in line with NMC Code of Conduct
  • Ensure medication audits are completed in line with company policies
  • Proactively research current and future care/specialised practices to ensure Opus Care Ltd is leading the way with care/specialised care delivery.
  • To actively engage with residents in conversation and meaningful occupations related


to their lifestyle choices at a level and pace that values the individual and respects their dignity and communication differences.


  • To respond in a timely way to residents who are distressed (calling out, calling for
help, knocking or making noises that suggest the need for support) or seek assistance if you feel unable to respond appropriately.

To be knowledgeable regarding emergency procedures pertaining the client group.


Health and Safety

  • Ensure the Registered Nurse, Clinical Lead Nurse is informed of any incidents in the absence of the Home Manager.
  • Ensure storage of medications are in line with company policies and procedures.
  • Ensure risk assessments are completed within the home and safe working practices are being followed.
  • Ensure equipment and the working environment are safe within the home.

Financial

  • To make sure clients have what is legally their property.
Reporting to the Home Manager or Clinical Lead Nurse and Registered Nurse.

The Community Lead is responsible for ensuring a high level of care is provided to our residents and meets the code of conduct and company procedures.


The Care Coordinator will support the Registered Nurse, Clinical Lead Nurse and the Home Manager providing leadership to the clinical team and providing strong clinical / specialised knowledge to ensure the highest standards are achieved in all areas of the home, including relationship centered care and clinical care.
