Description
Provide leadership and support for care teams on a daily basis

  • To work as part of management team within the home ensuring clear lines of communication
  • To provide and maintain continually high standards of care delivery within the home
  • To maintain and encourage a professional and collaborative work ethic with all team members and visitors to the home
  • Work with teams to coach and update on the organisations policies and procedures, legislation and CQC regulations ensuring good practice at all times.
  • To ensure that meaningful activities are provided for the people living in the home in partnership with the lifestyle coordinator and that people are encouraged and supported to remain as independent and active as possible.
  • Provide timely and focused hand overs to team members at the beginning of each shift to ensure outcomes are achieved which are relevant to that shift or specific to any of the people we are supporting.
  • Allocation of workload and location for the care team members on duty
  • To provide effective induction and training for more junior members of the team in line with the organisations policies and procedures and ensure this is recorded and evidenced.
  • Promoting effective and clear communication with care teams and people living in the home on a daily basis
  • Conduct and document care and risk assessments and develop care plans related to the assessed needs and choices of individual people living in the home.
  • Have a clear understanding of Safeguarding of Vulnerable Adults and the reporting process required.
  • To promote the requirements of the Mental Capacity Act and Deprivation of Liberty Safeguards in relation to consent and best interest decision making and ensure this is evidenced as required for residents.
  • Monitoring of quality assurance, implementing QA systems established within the home
  • Support the nurse in carrying out daily medication audits
  • Monitoring medication administration and documentation
  • To ensure that Team Leaders are competent to administer medication and have been inducted and assessed prior to medication administration in line with our policies and procedures.
  • Assisting and accompanying people who live in the home as required if examinations by Health Care Professionals are required
  • Liaising with all stakeholders involved with the people living in the home
  • Healthcare professionals, Social services, family members, etc to ensure that they receive the medical and clinical support they require.
  • Preparing for and participating in peoples reviews ensuring up to date information regarding the persons care package is up to date and available.
  • Maintaining current and appropriate documentation applicable to people living in the home.
  • Carry out appropriate clinical observations and record them accordingly
  • Carry out urinalysis testing as required for people living in the home
  • Monitor blood glucose level, as per their regime for people living in the home or in an event of an emergency
  • To assess pressure areas and carryout simple dressings of wounds as needed for people living in the home.
  • To attend and contribute to Management team meetings
  • Any other duties deemed appropriate in conjunction with the Home Manager
  • To complete supervisions and appraisals for care companions in line with CQC regulations and the organisations policies and procedures.
