Hi, my name is Kerry, and I am the Community Team Manager for our community team within Children, Family and Young People's services here at NSFT in Norwich.

We are creating the conditions to embrace change whilst always ensuring the basics.

Full time and part time applicants will be considered.


As Assistant Practitioner you will:


  • Support the registered nurse in the running of a shift
  • Assess service users' needs in conjunction with a registered professional.
  • Work independently on own initiative to meet the needs of services users.
  • Liaise and refer with external organisations as required promoting partnership and working to ensure holistic care.
  • Ensure service users' needs are accurately reflected in health record documentation.
  • Assist service users and carers to access information and support.
  • Support, promote and value equality and diversity.
  • Work well both independently and with the multidisciplinary team
