Description
We are looking for a highly motiviated and experienced Care Coordinator to join our Team at Belmont House.

the starting wage is £23,000, this will be reviewed at the end of the Probationary Period, which is 6 months.


We are a small family run business so it is important that you are friendly and approachable to reflect our culture.

If successful the Care Coordinator would report to the Manager and Deputy Manager and be accountable to the Manager.

Functions of the Role:


  • To provide support to people in a way that is respectful, promotes independence and enables the Residents to maintain, and improve on, their independence.
  • To always promote good practice and act as an effective and positive role model.
  • To proactively identify and work with the Residents and their Families to coordinate and navigate their care and support whilst they are living at Belmont House.
  • To work closely with GPs, District Nurses, and other Professionals to ensure the effective management of the Care and Support of the Residents at Belmont House.
  • To support Residents to understand and manage their condition and to ensure their changing needs are addressed.
  • To as a central point of contact to ensure appropriate support is made available to them and the carers providing their daily Care and Support.
  • To carry out duties delegated by the Manager and Deputy Manager, including the delegation of duties to Carers.
  • To ensure Residents rights are protected and that they are offered choice.
  • To coordinate activities in the Home and ensure that these are meaningful and staff interactions are appropriate.
  • To ensure that each Resident is supported in a personcentred way by adopting a positive approach to person centred planning and delivery to ensure that the team are not providing institutional support.
  • To ensure that the Care Plans of the residents are followed by the Care Staff.
  • To promote the wellbeing of each individual Resident.
  • To attend meetings as arranged by management.
  • To be proactive in communication through the Communications Book and Handover.
  • To attend supervisions as required.
  • To lead the shift allocated in a professional manner and to ensure that the team understand their responsibilities and delegated tasks.
  • To maintain the safety of individuals under your supervision.
  • To understand how and when to obtain help and advice.

Key Areas of Care Coordinator Role

  • In conjunction with, and under the instruction of the Deputy Manager and Registered Manager, assisting with the daytoday supervision and management of the Home. This will include allocating work tasks, ensuring the completion of training and participating in the induction of new staff.
  • Communicating with Residents using appropriate communication methods, including tact, empathy, and diplomacy.
  • Assisting with individuals' care, including personal care, and assisting with bathing to maintain a high standard of hygiene where the individual requires this and delivering this ion a personcentred way. This may include people moving techniques, using hoists, and repositioning Residents as required.
  • Participating in the writing, development, and implementation of Residents Care Plans to encourage the Residents to take an active role in all aspects of daily living and ensure the appropriate level of individual care and support is identified.
  • Work with the Residents, their families, and carers to improve their understanding of the Residents condition and support them to be involved in the development and review of the Resident's Personal Care Plans through completed Quarterly Person-Centred Reviews.
  • Planning, holding, and documenting Quarterly Person-Centred Reviews and ensuring that all actions and documented and completed.
  • Work collaboratively with GPs and other primary Care Professionals to proactively identify and manage the physical and emotional wellbeing of the Residents.
  • Ensuring that all activities such as Personal Care and Medication Administration meet the required standards.
  • Complete Mental Capacity Assessments for the Residents who require Bed Rails and Sensor Mats.
  • Ensuring that all Residents are engaged in activities that are beneficial to them and are of their choosing.
  • Support practices to keep care records up to date by identifying and updating missing or outofdate information about Residents circumstances.
  • Recording and completing any appropriate documentation to ensure that Residents records and observations are accurate and informative.
  • Contribute to risk and impact assessments, monitoring, and evaluations of the service.
  • Checking all the daily documentation for Residents to ensure that it has been completed accurately and effectively.
