Stroke Advanced Clinical Practitioner (37.5 hours a week) Ward/ Rehabilitation

The post holder will have a Trust-wide responsibility to promote clinical excellence in the care of patients presenting with Stroke by providing clinical advice and intervention, with support to therapy, nursing staff and medical teams.

They will provide expert professional and independent clinical care which enables the coordination of a multi professional seamless service for these patients.


The successful applicant will actively be involved in/lead ward rounds, lead MDT meetings, and act to facilitate a seamless transition for stroke patients with our community partners, using up to date clinical evidence of rehabilitation methodology.

Louth Hospital is set on the edge of the Wolds and serves the communities of Louth and the local area.

It provides a number of services, including an outpatient department, diagnostic and surgical procedures, as well as a surgical ward.
