The Integrated Discharge Team are seeking an individual who is dynamic, flexible and self-motivated, with experience of planning and delivering service transformation initiatives.

As an improvement lead you will work to support the Assistant Director of Operations in the Home First Team with service improvement


Our vision is:

Enabling patients to access hospital services when they need it through facilitating patient discharges in a safe and supportive way.


You will work with a range of clinical and non-clinical teams to establish and deliver key initiatives to improve patient care and patient /staff experiences relating to patient discharge from hospital.

You may also be involved in Trust-wide initiatives.

  • Assist in the development of an improvement culture which supports the highest quality and safety of patient care.
  • Engage, influence and work collaboratively with senior managers, corporate services, operational management, clinical and administrative teams to deliver sustainable improvements across the organisation
  • Provide expertise to review processes and systems, and develop solutions to resolve issues
  • Provide expertise to initiate, plan and deliver improvement initiatives that will improve the quality of care delivered to patients while enhancing the overall patient and staff experience.
  • Lead allocated Transforming Care programmes in conjunction with senior responsible officer and project team
  • Provide improvement, innovation and service redesign expertise to allocated Division, working with the senior Divisional management team to ensure alignment with strategy and operational priorities.
  • Support and deliver projects within cost improvement programmes, when required
  • Support the building of capability across the organisation, enabling staff to use quality improvement, change management and project management skills to successfully make improvements within their areas of work
  • Collaborate with service users, carers, Trust governors and external partners to shape improvement initiatives, where appropriate.
  • Work autonomously without direct supervision
  • Be able to recognise and respond to the human dimensions of change that may arise as a consequence of improvement initiatives
