The Provide Stroke Rehabilitation Service supports 16 inpatient Stroke beds and a well established Early Supported Discharge Service.

We work in partnership with the Acute Stroke Units, Social Care and the Stroke Association providing seamless, high quality stroke rehabilitation to the people in our care.

Our Nursing and Therapy teams, supported by Stroke Psychologists and Medical Practitioners provide a progressive service which showcase true multi and interdisciplinary working.

We are looking for a motivated and dynamic Therapy Assistant to join our team.

The post holder will work as part of an integrated multi-disciplinary team of staff within the
Stroke Service.
To assist the qualified therapists in delivering an excellent multi-disciplinary therapy

service (Physiotherapy, Occupational Therapy, Speech & Language Therapy) to patients in their own homes and in the community hospital Stroke Rehabilitation Unit.

To assist in the organisation's clinical governance action plans in developing the service.
To contribute to the assessment and treatment of patients both individually and in groups.
To support the Therapists in the implementation of treatment plans with patients who have had a stroke.
To assist in the provision of equipment to patients in their own homes.

To assist the Therapists in promoting a patient's return to independence in their home environment, thus avoiding hospital admission, re-admission or long-term residential or nursing care.

To communicate with patients, relatives, carers, equipment services, social and health care professionals and other voluntary organisations and statutory bodies to assist the Therapists in ensuring an integrated, holistic approach to patient care.

Clinical

  • To liaise with other members of the multi-disciplinary team on behalf of the Therapist regarding a patient's Therapy.
  • To participate and contribute to the Therapist's assessment and treatment planning.
  • To complete delegated Physiotherapy/Occupational therapy/Speech & Language Therapy interventions. This may include personal care, domestic activities, group work and exercise programmes.
  • To appropriately measure, supply and adapt mobility aids and equipment.
  • To monitor patients/clients progress, taking account of their environment and adjust own clinical interventions accordingly and report back to the Therapist.
  • To liaise with the Therapist as necessary regarding patients therapeutic interventions particularly highlighting any variations to expected outcomes from treatment programmes you deliver.


Under the direction of the Therapists to be responsible for liaising with patients, relatives, carers, equipment services, social and health care professionals and other voluntary organisations and statutory bodies.


  • To encourage patients/clients and carers in an active approach to regaining personal independence using empathy, sensitivity, and highly skilled interpersonal skills.
  • To demonstrate therapeutic techniques to carers and families.
  • To assist in identifying appropriate equipment and adaptations required in patients own homes and under supervision, order equipment identified by Therapists and contribute to ensuring timely delivery.
