Description

The post holder will be one of four Digital Facilitators (DFs), who will work as a team across the Kent and Medway (K&M) system, working through the Health and Care Partnerships (HCPs) to support the Primary Care Networks (PCNs) with data quality, optimisation of systems, in particular, EMIS, Ardens, Apex, and Digital Front Door and to support general practice in basic data analytics.

As part of the Integrated Care Board (ICB) Digital First Roadmap programme, this 12 months pilot project will support general practice in K&M to standardise the recording of practice activity, especially GP activity, to ensure that when reviewing workload and access there is a visible, complete, and commensurable data set across all practices.

The postholder will work collaboratively with other DFs, managers, and clinicians to establish and share best practice, standardising where appropriate and beneficial, and supporting PCNs in developing the use of data.

The role scope also extends to other projects that support the use of information in the planning and delivery of care in general practice, including remote working, remote monitoring of patients, including digital support for care homes, as well as digital communication and messaging.
